Autologous Biocellular Products

The continued expansion of Regenerative/Biocellular Medicine and/or therapy is really much like the early beginnings of MSK ultrasound. Lots of interest because of the novelty and promise of a new and exciting imaging tool that could rival MRI… the “gold standard“ of medical imaging.

Regenerative Medicine is not necessarily new. It has been used for quite some time in the form of PRP (platelet rich plasma) by spraying it inside a surgical site before closure. Quite simply it promotes better healing.

As in the the early days of MSK ultrasound, there was lots of interest and excitement, but a lack of standardized procedures.

Regenerative Medicine is in a similar growth pattern. There is broad opinion on which Biocellular product is most effective and safe.

Non-autologous (cells or tissue NOT obtained from the same individual) products claim and present research that viable, living “stem cells” are present.

However; there is also a notable counter position as to whether undifferentiated, multi-potentiated cells are present in non-autologous products.

There is some debate as to which autologous product (adipose or bone marrow) is more effective, but neither of these products are subject to scrutiny about the presence of true regenerative properties.

My position is that bone marrow extract is more hemopoietic… more capable of producing blood cells than chemotaxic activity leading to ligament, tendon, hopefully cartilage regeneration.

Autologous products have essentially no opportunity for rejection when used in musculoskeletal/orthopedic applications.

Considering PRP (platelet rich plasma) there is variability in commercial kits and even self produced PRP.

I would strongly recommend using the Emcyte Pure PRP system to achieve a reproducible truly platelet RICH plasma product.

Lastly; absolutely every regenerative medicine procedure requires employment of real time ultrasound at time of treatment.
